About the Author
John N. Briere, PhD, is Associate Professor of Psychiatry and Psychology at the Keck School of Medicine, University of Southern California, where he consults and teaches in the ER, burn unit, and inpatient psychiatry, and Center Director of the USC Adolescent Trauma Training Center’s National Child Traumatic Stress Network. A past president of the International Society for Traumatic Stress Studies, he is recipient of the Robert S. Laufer Memorial Award for Scientific Achievement from ISTSS, and the Award for Outstanding Contributions to the Science of Trauma Psychology from the American Psychological Association’s Division 56, and has been designated Highly Cited Researcher (Behavioral Sciences) by the Institute for Scientific Information. He is author of a number of books, articles, and psychological tests in the areas of trauma and interpersonal violence, and has a long-standing interest in the clinical applications of meditation, mindfulness, and compassion. What a multiply traumatized young adult sees in this book

I *believe* that this book would be a very worthy addition to any trauma clinician's library. However, unfortunately I am not a clinician myself so I cannot assess the quality of the material with regards to clinical practice, how it incorporates theoretical frameworks, etc. "Then why are you writing this review?" Well, for one I think the scholarship probably speaks for itself. But the real reason is that I went and read the sample thinking that this was a book for the patient to read, and I felt like since so little has been said about it thus far I might as well introduce it a little and throw in my two cents as a young adult CT patient.
-Why did they write this book?
"Unfortunately, although complex trauma and its effects are common, there are few empirically informed treatments specifically developed for multiply traumatized children or adolescents. This is partially due to the challenging nature of the problem - the range of these impacts often requires a multimodal, multicomponent treatment strategy. Treatment approaches that are limited to a single modality (e.g., exposure therapy, cognitive therapy, or psychiatric medication) can sometimes be insufficient - especially if the intervention approach is not adapted to the specific experiences, psychological needs, and cultural matrix of the affected youth." This book is an attempt to outline such a multi-modal, intergrative approach (ITCT-A) based on the latest experimental findings.

Here's an overview of the basic treatment model (ITCT-A) endorsed by this book:
Overview of ITCT-A:
*Assessment-driven treatment, using an interview-based symptom review measure (the Assessment-Treated Flowchart; see Chapter 4) and, when possible, trauma-specific tests administered at three-month intervals
*Attention to complex trauma issues, including posttraumatic stress, behavioral and affect dysregulation, and interpersonal difficulties
*Customization, involving application of different treatment components for each client, based on his or her history, needs, symptoms, and cultural context
*Multiple treatment modalities, including cognitive therapy, exposure therapy, affect regulation training, and relational treatment in individual, group, and caretaker therapy
*Focus on a positive working relationship with the therapist, deemed crucial to the success of therapy for complex trauma
*Attention to attachment difficulties, associated with early, developmental trauma
*Cultural adaptations of treatment components to maximize their relevance to clients from different social and ethnocultural groups
*Early focus on immediate issues such as acute crisis and self-endangering behaviors
*Skills development, in terms of building emotional regulation and problem-solving capacities
*Titrated therapeutic exposure and exploration of trauma within a developmentally appropriate and safe context, balanced with attention to the client's existing regulation capacities
*Advocacy and interventions at the system level to establish healthier functioning and to address safety concerns
*A flexible time frame for treatment, since the multiproblem nature of complex trauma sometimes precludes short-term therapy
And here's what some of that sounds like to me, as an otherwise unqualified young adult CT patient, who has seen a handful of apparently capable trauma therapists with no luck (as in, I stopped seeing them because they weren't helping me):
-Assessments? I actually love assessments. It helps me feel better (and I imagine the clinician too) to have a test with a presumably plug-and-chug assessment of what we're looking at, in light of the current research. I think these assessments are likely a major strength and pulling point of this book.
-Taking posttraumatic stress truly into account and emotional and social problems seriously should be a no-brainer but unfortunately this is not always the case. I imagine this one's going to rely in large part on the specific capabilites of the therapist, not all cases being equal.
-Especially, EARLY FOCUS on immediate issues and self-harming behaviors. Yes!!! Absolutely!!! Not "we'll get to it" or "I'll assume things."
Like, to give you an idea of where I'm coming from, I've seen at least three therapists who KNEW I was currently dealing with especially severe depression and I approached all of them upfront telling them I believed I was a complex trauma case. And NONE of them 1) assessed me for suicidality 2) one of them actually forgot I was depressed 3) if I dropped hints or exhibited obvious signs of distress (and dissociation, mind you), I felt like my immediate needs were going thoroughly unnoticed/unhelped. Like presumably "we'll get to it." But no, I don't particularly want to talk about/process my trauma experiences some more, at the moment. I've already said what I want to say. What I need is something that can help me NOW. Or at least something I can refer to between now and our next appointment to help me cope and make progress between visits.
-And not to mention, they also didn't give me a TIME FRAME. Like, am I going to be in therapy for the rest of my life? Of course this type of thing doesn't happen overnight, but what exactly are we working on that's going to take so long? What can I expect? Is this even going anywhere? Because I'm starting to feel pretty hopeless about this...
-(And, this is where we could really use that "balance with attention to the client's existing regulation capacities" part...)
So from such a patient as this book is intended to be used with, this all sounds very good to me. Coming from abusive and traumatizing situations and already feeling like "no one understands me, I can't be heard!" I can't tell you how frustrating it is to get this same kind of "they're not even listening to my input!" feeling in the clinician's office! Quelle horreur! So maybe this adaptive and customizable (note: I read this as "listening and sensitive") approach is worth your consideration...
So yes, if you're a clinician out there considering buying this book, then good for you! You should probably go for it. Unfortunately my assessment ends there and I can't vouch for the material beyond that.
If you're a patient and looking for some self-help, this isn't the book's intended purpose. (Notice the title, *Treating* Complex Trauma.) However, you could always give a copy to your therapist and utilize it in therapy.
Statement of purpose:
"This is not a 'how-to' manual. Instead, we offer a semi-structured approach that can be adopted on a case-by-case basis by the therapist in order to meet the youth or young adult's specific development level, psychological functioning(!), and cultural/ethnic background."
So I think this book is a definite step in the right direction. Hence the five stars.
